What should I write down after a consultation?

Record the decision in ordinary language

MedlinePlus recommends taking notes and asking for written instructions when needed. After an Rx2BFIT assessment, write down the main concern discussed, what the provider believes may be contributing to it, and why a particular next step was recommended. Distinguish something the clinician proposed from an option you merely discussed. If a test or record review is still pending, note that the recommendation may be provisional.

Include the alternatives that were considered and the reason each was accepted or set aside. If you heard a broad statement about expected results, ask the clinician to explain what was meant for your circumstances and what remains uncertain. A written note should not turn a conditional discussion into a guaranteed outcome.

Capture responsibilities and timing

Write down who will order tests, review results, contact you, and schedule the next check-in. Note what observations you are expected to track and which concerns should prompt earlier contact. Ask for the office’s actual communication route rather than assuming an informal message will reach the right person.

Keep clinical notes alongside, but separate from, payment terms. If the written plan and quote differ from your understanding, seek clarification before starting. A concise record makes later reassessment more useful: both you and the clinician can see what the original goal and next decision were. It also helps you share the plan accurately with another provider involved in your care.

If a family member or support person attended, compare notes while the conversation is fresh. Where memories differ, send a concise clarification question to the office instead of relying on either recollection as the definitive plan.
